Utilize this document to streamline the interview process and to help reduce the stress of interview season. Print and insert the template into a binder. Its purpose is to be used for interview preparatory work, notes during interview, and impression after the interview. Maintain an accurate record of contact information and thank you notes with the follow-up tracker.

I am providing a template that I created to stay organized as I applied to residency programs. I had 18 interviews for which I needed to research each program, write questions to ask every interviewer, organize the responses to those questions, document my impression of each program, organize all of the contact information provided, and keep track of the thank you emails that I needed to send to each interviewer.
Not only that, there were virtual meet and greet sessions hosted by many of the programs. Most of these meet and greets were hosted by residents and were much more informal than the interviews. I needed to have questions prepared for those sessions. I also wanted to keep the residents' contact information organized so that I could send each of them thank you emails and follow-up with questions in order to show my interest in their program.
For every residency interview that you schedule during the interview season, print out this template and put it in your binder. This binder should contain separate sections for each residency program. I managed to keep everything organized by developing this template. I know that it will be just as beneficial to you as it was to me.
